COVID_19 Vaccination Card Upload - Considering the current City of Chicago guidelines, all meeting attendees will be required to show proof of vaccination prior to check-in at the registration desk and  badge pick-up. In order to avoid long lines during registration, attendees have the option to voluntarily upload proof of vaccination using the upload option below.

Acceptable forms of proof (scan or photo is acceptable for upload)*:

  • Vaccination Card 
  • Digital record or app 
  • Printed record from vaccine provider

*Please ensure that the file size is less than 2MB and that it is either in a .pdf or .jpg format.
