Surviving the Peri-Implant Maelstrom: Efficient Treatment Strategies for the Failing Implant

Abstract:
The increasing incidence of peri-implantitis has spurred both scientific and clinical research aimed at gaining a deeper understanding of this complex pathology and developing more effective treatment strategies. Among the various surgical treatment options, implantoplasty has attracted growing attention. This technique involves the removal of both macro- and micro-structures, such as threads and surface treatments, from the exposed implant surface using rotary instruments. It has been traditionally associated with resective procedures but this presentation will demonstrate how implantoplasty, when used alone, can promote bone repair and, when combined with regenerative techniques, can be part of a combined treatment approach. Long-term clinical results will be presented, showing that, in all situations, implantoplasty emerges not only as an adjunct but also as a potentially indispensable component of comprehensive peri-implantitis treatment strategies. However, concerns regarding technical difficulties, the potential dispersion of titanium particles, and the risk of implant fracture have been raised. These critical issues will also be addressed.
